Patient Forms + Office Policies

We are very excited to welcome your family to Parker Place! You may find that our new patient forms are a little more detailed than those at other practices - this is because we want Dr. Parker to have the most accurate picture possible of your child’s health and to be able to make the most of her time with your family.

We highly encourage you to complete your child’s new patient forms online, and securely submit them at least 48 hours prior to your scheduled appointment time. By doing so, you can plan to arrive just 5-10 minutes before your scheduled appointment time, and Dr. Parker can review your child’s medical history and any current concerns in advance.

If you prefer, you can also download a printable copy of our new patient forms to fill out and bring with you - if choosing this option, please plan to arrive about 20 minutes before your child’s appointment to allow our staff time for some necessary data entry. If you are unable to complete the new patient forms in advance and need to fill them out in our office, we do ask that you arrive 45 minutes before your scheduled appointment time.

Inclement Weather

Parker Place determines its operating status based on weather conditions and staff availability. In the event of inclement weather, please call our office at 410-844-8998 to confirm whether we are open or closed. 

Lateness, Missed Appointments & Cancellations

Our goal is to see every patient on time, and this requires that we maintain an accurate appointment schedule. Please note that we reserve the right to reschedule any patient who arrives 15 or more minutes late for a scheduled appointment. In addition, if you have chosen not to complete your new patient forms in advance of your arrival, you must arrive at least 20 minutes before your scheduled appointment time in order to complete this paperwork and be seen by the doctor on time.

Please note that missed appointments (no-shows) will incur a $50 fee that will billed directly to the responsible party, not to insurance.

If you need to cancel a scheduled appointment, we ask that you do so at least 48 hours prior to your appointment time. Cancellations made within 48 hours of the appointment time will be considered missed appointments (no-shows) and may incur a $50 missed appointment fee.

Questions for the Doctor

In the event of any medical emergency, please call 911. For non-emergency medical questions, day or night, please utilize the messaging feature on our online patient portal and we will respond as quickly as possible. If you prefer to leave a message directly with our staff, you can call the office at 410-844-8998 during business hours and leave a message with our staff, so that Dr. Parker can return your call. If you have a medical question after hours, you can also call this same number after hours and leave a voice mail with your callback number. We make every effort to check and respond to voice mails and messages as quickly as possible on the following business day.

Children in the Waiting Room

We strive to make our office a fun, engaging place to visit. To ensure your children’s safety and limit disruption to other families and visitors, we ask that you bring all children under the age of 12 into the exam room with you, unless there is another adult present to supervise them.


In an effort to help older children – generally, those aged 13 and up – feel involved and invested in their health care, Dr. Parker prefers to spend part of each appointment meeting with the child independently, with parents in the waiting room. Parents will be invited to join their child and Dr. Parker toward the end of the visit for a brief review of what was discussed, any follow-up care necessary, etc. If you have any questions or concerns, we invite you to speak with Dr. Parker or any member of our staff.

Insurance & Payment

Parker Place accepts most commercial insurances and state-supported plans. To confirm our participation in your plan, please contact our office directly. In addition, please be prepared to provide your current insurance card and photo ID at each visit for verification purposes.

You are responsible for paying any required co-pays at the time of your appointment. Our office will then submit a claim to your insurance company on your behalf, and any remaining balance (whether due to denied claims, non-covered services, or deductibles not met) will be billed to you. Payment for this billed amount must be submitted in accordance with the due date. Should our office be required to pursue additional collection efforts, you agree to be responsible for any incurred fees.

We also offer a self-pay fee schedule for families who opt to pay out-of-pocket for their child’s medical care. Please let us know at the time of your appointment if you will require documentation to submit to your insurance company for reimbursement.

Please note that, for any other medical services required for your diagnosis or treatment (such as lab work or imaging), you are responsible for paying those service providers directly in accordance with their payment policies and your insurance plan.

For co-pays and additional amounts owed, Parker Place accepts major credit cards (Visa, MasterCard, Discover and American Express), personal checks, and cash.

Updates to Policies

Please note that our policies may be revised periodically as the need arises. We will notify you of any changes by providing you with an updated Medical Office Policies document upon your next visit to our office. If you have any questions regarding our policies, you are encouraged to contact our staff at any time.

Acknowledgment of Receipt

We request that each patient’s parent or guardian confirm receipt of our office policies, as well as other important Parker Place information, by signing the separate Acknowledgments & Opt-In document included as part of our new patient forms.
